Sullivan Health Access, Inc.

Our mission, what we will offer, and answers to your questions

Where you live should never determine the quality of care you receive.

Our Mission

Sullivan Health Access, Inc. is a New York nonprofit and a tax-exempt 501(c)(3) charity, founded on a simple belief — that where you live should never determine the quality of care you receive.

Our mission is to make lasting improvements in access to primary health care in Sullivan County through community-driven solutions and partnerships with care providers and other organizations.

To implement that mission, we are opening Highland Health Access (HHA) — a health hub in Barryville that will serve the medical and health-related social needs of individuals in the Town of Highland and nearby communities.

What Highland Health Access aims to offer

Health screenings
We plan to offer a trained team of community health workers and registered nurses, along with nurse practitioners and physician assistants, who will check basics like blood pressure, blood oxygen, and heart rhythm (ECG), and ask about your everyday needs such as food and transportation.
Connections to care and support
Based on your results, we plan to connect you directly with comprehensive medical care from providers such as Sun River Health. We will also connect you with help for food, transportation, mental health counseling, and more, through organizations such as Sullivan Allies Leading Together (S.A.L.T.) and the Hudson Valley Social Care Network.
A personal care plan
We will work with you to build a care plan based on your health information and goals, then provide dedicated, ongoing support, checking in regularly — more often when you need it. We plan to have staff in office several days a week, and to offer home visits as necessary, as well as virtual (online) visits.

Your Highland Health Access questions, answered

Will I be able to see a doctor?
Yes, part-time. We plan to have a visiting physician in the HHA office about one week each month. Between physician visits, our team can connect you with other medical providers — and if you do not have a regular doctor, we will help you find one.
Will I be able to get a prescription filled?
We do not presently plan for HHA to have a pharmacy or be able to fill prescriptions, but when our physician is in the office they can write or renew prescriptions for you.
Will you take insurance? What will it cost?
We are still finalizing costs and payment options, including for people with Medicaid, Medicare, private insurance, or no insurance. We will share details before we open.
Where will it be, and when will it open?
It will be located in the former medical office at 3452 State Route 97 in Barryville, renovated with support from the New York State Assembly through the Town of Highland. We are also seeking further federal, state, and foundation funding, and will announce an opening date as soon as possible.
Is it for emergencies?
No. Highland Health Access is not an emergency room or urgent care center. In an emergency, please call 911.
